How to view assessments in PC

1. From the Precision Care “home screen” click Menu, scroll down to assessments. To the right, click assessments again under Forms.

2. Choose person in service, select the program of the individual you wish to view. A roster will populate of the individuals within that program. Select the individual.

3. On this screen you have the ability to search by date range as well as filter by the type of assessment you are looking for by using the drop-down box. Choose print icon to view selected assessment.
